The mechanisms are well understood. Sustained near focus requires continuous contraction of the ciliary muscle (accommodation) — producing ciliary muscle fatigue that manifests as blurred vision and eye ache. Screen use reduces blink rate from the normal 15–20 per minute to 5–7, causing tear film instability and ocular surface drying. The forward head posture of screen work creates cervicogenic eye strain through the myofascial and neurological connections between the cervical spine and the ocular muscles.

Standard management — the 20-20-20 rule, blue light glasses, ergonomic adjustment — reduces the strain but does not treat the accumulated ocular and cervical tension that develops in heavy screen workers.
