Why PPIs Manage Reflux But Don't Resolve It

Gastro-oesophageal reflux disease (GERD) occurs when stomach acid refluxes into the oesophagus, producing heartburn, regurgitation, chest discomfort, and in chronic cases, oesophageal damage. The primary mechanism is dysfunction of the lower oesophageal sphincter (LES) — abnormal transient relaxations allowing acid to escape upward.

Proton pump inhibitors (PPIs) — omeprazole, pantoprazole — suppress acid production effectively. But they do not correct the LES dysfunction driving reflux, do not address the delayed gastric emptying that contributes to it, and carry risks with long-term use including microbiome disruption, magnesium deficiency, and increased fracture risk. Many patients find their reflux returns or worsens when they try to stop PPIs — "rebound acid hypersecretion."

TCM Perspective

In TCM, acid reflux is Stomach Qi rebelling upward — the Stomach's normal descending function disrupted, causing its contents (including acid) to move in the wrong direction. This may arise from Heat in the Stomach (the most common pattern), Cold invasion, Liver overacting on the Stomach (stress-triggered reflux), or Phlegm-Heat obstruction.
