Understanding the Condition

The Most Common Benign Tumour in Women — and Why Management Is Often Inadequate

Uterine fibroids (leiomyomas) are benign smooth muscle tumours of the uterus affecting an estimated 70–80% of women by age 50, though most are asymptomatic. Symptomatic fibroids produce heavy menstrual bleeding, prolonged periods, pelvic pain and pressure, urinary frequency, and subfertility — significantly impacting quality of life.

Standard management is either watchful waiting (for small, asymptomatic fibroids) or surgical intervention — myomectomy or hysterectomy. Medical management with GnRH agonists reduces fibroid size temporarily but produces menopausal side effects and is not a long-term solution. Many women find themselves in an inadequately managed middle ground: too symptomatic to wait, not yet ready for surgery.

Fibroids are oestrogen-dependent — they grow in the presence of oestrogen and regress after menopause. This hormonal dependency is the mechanism through which acupuncture's oestrogen-modulating effect can influence fibroid behaviour and, more reliably, fibroid-related symptoms.

TCM Perspective

In TCM, fibroids are Blood stasis masses — zheng jia — formed by the accumulation of stagnant Blood in the uterus over time, combined with Phlegm-Damp in many presentations.

Mechanism of Action

How Acupuncture Manages Fibroid Symptoms

Heavy bleeding reduction

Acupuncture at SP-1, SP-10, CV-4, and BL-17 regulates uterine haemostasis and reduces the excessive endometrial proliferation that causes heavy fibroid-associated bleeding. This is often the most impactful effect for patients — reducing blood loss significantly within 2–3 cycles.

Pelvic pain and pressure

Blood stasis treatment reduces the pelvic congestion and inflammation contributing to fibroid-associated pain. SP-8 and LV-3 are key points for pelvic Blood stasis.

Oestrogen modulation

Acupuncture influences aromatase activity and oestrogen receptor sensitivity — creating a less oestrogen-dominant hormonal environment that reduces the stimulus for fibroid growth.

Phlegm-Damp resolution

Where Phlegm-Damp contributes to the fibroid pattern (common in larger fibroids with a soft consistency), treatment includes ST-40 and SP-9 to resolve the Phlegm component.

Evidence Base

What the Research Says

Clinical evidence on acupuncture for fibroids focuses primarily on symptom management rather than fibroid size reduction. Studies consistently show acupuncture significantly reducing menorrhagia (heavy bleeding) and pelvic pain in fibroid patients — with outcomes comparable to or better than NSAIDs and tranexamic acid. For fibroid size, Chinese hospital studies report modest size reduction with extended acupuncture courses, though this is not the primary therapeutic claim. The WHO lists uterine …

Patient Experience
My periods were so heavy I was becoming anaemic. My gynaecologist had recommended surgery but I wanted to try other options first. After 3 months of acupuncture with our team my periods reduced from 8 days to 5 and the flooding stopped completely. My iron levels normalised.
