Why Bacteria End Up Where They Shouldn't Be

Small intestinal bacterial overgrowth (SIBO) is the abnormal presence of excessive bacteria in the small intestine — a segment of the gut that should contain relatively few microorganisms compared to the colon. When bacteria colonise the small intestine in significant numbers, they ferment carbohydrates and produce gas (hydrogen, methane, or hydrogen sulphide) before normal digestion can occur. The result is the characteristic SIBO symptom picture: severe bloating and distension within 30–60 minutes of eating, gas, altered bowel habit (diarrhoea in hydrogen-dominant SIBO, constipation in methane-dominant SIBO), nutrient malabsorption, and chronic fatigue.

SIBO is diagnosed by breath testing — measuring hydrogen and methane gas produced by bacterial fermentation. Standard treatment is antibiotic therapy (rifaximin, neomycin) targeting the overgrowth — effective in the short term but associated with high recurrence rates, because the antibiotic addresses the bacteria without correcting the underlying motility dysfunction that allowed SIBO to develop.

The migrating motor complex (MMC) — the intestinal "cleaning wave" that sweeps bacteria from the small intestine between meals — is impaired in most SIBO patients. This impairment is the primary reason SIBO recurs after antibiotic treatment. Without restoring MMC function, bacterial overgrowth returns within weeks to months.
