Plantar fasciitis is inflammation and degeneration at the origin of the plantar fascia — the thick fibrous band that supports the arch of the foot. The classic symptoms: sharp pain in the heel and arch on the first steps of the morning, easing after a few minutes, then returning after prolonged standing or walking.

It affects runners, people who stand all day, those who are overweight, and — increasingly — desk workers whose sedentary lifestyle has weakened foot intrinsic musculature. In Mumbai's climate, flat sandals and flip-flops (which provide no arch support) make it even more prevalent.

TCM Perspective

The frustrating reality of plantar fasciitis is that the standard advice — rest, stretching, insoles — helps but rarely resolves it. The plantar fascia is under load during every waking hour, making true rest impossible. And unlike muscle injuries, fascial tissue has poor blood supply and therefore slow healing capacity. How Acupuncture Heals the Plantar Fascia

Direct fascial treatment

Needling into the plantar fascia at its calcaneal origin and along its course stimulates fibroblast activity and collagen remodelling — the biological repair process that passive rest cannot initiate.

Intrinsic muscle release

Flexor digitorum brevis, abductor hallucis, and the deep intrinsic foot muscles harbour trigger points that increase plantar fascial tension. Needling releases these, reducing the load on the fascia.

Gastrocnemius and soleus treatment

Tight calf muscles are a major contributing factor to plantar fasciitis by increasing the pull on the Achilles-plantar fascial complex. Acupuncture releases calf trigger points and restores normal length.

Kidney channel treatment

In TCM, the plantar fascia and heel fall within the Kidney meridian's territory. Kidney deficiency — common in middle-aged and older patients — creates constitutional vulnerability. KD-1, KD-3, and SP-6 address this pattern.

A 2017 RCT (BMJ Open) found acupuncture equivalent to cortisone injection at 4 weeks and superior at 12 months — with a dramatically lower recurrence rate. A 2021 systematic review confirmed acupuncture's superiority over standard care for chronic plantar fasciitis. The WHO lists foot conditions as indications for acupuncture.
