The Most Common Joint Condition — and Why Pain Management Alone Is Insufficient
Osteoarthritis (OA) is the progressive degeneration of articular cartilage, producing joint pain, stiffness, reduced range of motion, and crepitus. In India, OA affects an estimated 15% of the population and is the leading cause of disability in older adults. Knee OA is the most prevalent, followed by hip, hand, and spinal facet joint OA.
Standard management is a stepwise escalation of pain relief: paracetamol, NSAIDs, cortisone injection, hyaluronic acid injection, and ultimately joint replacement. This approach manages pain but does not address the central sensitisation that amplifies OA pain, the muscle weakness that accelerates joint loading, or the systemic metabolic factors that drive cartilage breakdown.
Acupuncture's role in OA is not cartilage restoration. It is consistent, significant pain reduction — addressing the neurological, inflammatory, and muscular components of OA pain in a way that improves function and quality of life, often reducing the need for analgesics and deferring or avoiding the need for surgical intervention.
In TCM, OA is understood as Kidney and Liver deficiency (the constitutional root) combined with Bi syndrome obstruction (the symptomatic expression). Treatment addresses both.

Central pain modulation
OA pain is significantly amplified by central sensitisation — the nervous system's learned hypersensitivity to joint pain signals. Acupuncture downregulates this central amplification, reducing pain out of proportion to the structural damage.
