When Blood Pressure Is Too Low to Perfuse the Brain Adequately

Hypotension — blood pressure chronically below 90/60 mmHg — produces symptoms through inadequate cerebral perfusion: dizziness, lightheadedness, fatigue, blurred vision, difficulty concentrating, and in severe cases, syncope (fainting). Orthostatic hypotension — a drop in blood pressure of ≥20 mmHg systolic on standing — is particularly debilitating, making standing, walking, and exercise problematic.

The causes include autonomic dysfunction (the most common mechanism in young, constitutionally thin patients — the body's autonomic response to positional change is insufficient), dehydration, medication side effects, adrenal insufficiency, and cardiac causes. In young women with low body weight, constitutional Qi and Blood deficiency is the typical pattern.

Standard management — increased salt and fluid intake, compression stockings, fludrocortisone — addresses symptoms rather than the underlying autonomic and constitutional deficiency.

TCM Perspective

In TCM, hypotension reflects Spleen and Kidney Qi deficiency (insufficient Qi to maintain vascular tone), Heart Yang deficiency (the heart lacks the force to maintain adequate pressure), or Qi and Blood deficiency (the most common pattern in young women — insufficient substance to fill and pressurize the vascular tree).
