Understanding the Condition

PMS Is Not Normal — Even Though It's Common

Premenstrual syndrome affects an estimated 75% of menstruating women to some degree, and 20–30% experience symptoms severe enough to disrupt daily functioning. The cluster of symptoms — mood changes (irritability, anxiety, low mood), physical symptoms (breast tenderness, bloating, headache, fatigue), and cognitive symptoms (brain fog, poor concentration) — appears in the luteal phase and resolves with menstruation.

PMS is driven by abnormal sensitivity to the normal hormonal fluctuations of the luteal phase — particularly the drop in oestrogen and progesterone in the days before menstruation. Women with PMS do not have abnormal hormone levels; they have an abnormal neurological response to normal hormonal changes, involving serotonin dysregulation, altered GABA sensitivity, and HPA axis reactivity.

TCM Perspective

In TCM, PMS is primarily Liver Qi stagnation — the Liver's failure to regulate Qi flow in the lead-up to menstruation, producing the emotional irritability, breast distension, and physical tension characteristic of the condition. Depending on the specific pattern, Blood deficiency, Heart Shen disturbance, or Spleen Qi deficiency may contribute.

Mechanism of Action

How Acupuncture Relieves PMS

Liver Qi regulation

LV-3, GB-34, PC-6, and LV-14 address the Liver Qi stagnation pattern directly — reducing the emotional reactivity, irritability, and breast tenderness that characterise the pre-menstrual phase.

Serotonin regulation

Acupuncture upregulates serotonin availability in the luteal phase — addressing the neurochemical deficiency that drives PMS mood symptoms, via the same pathway as SSRIs but without their side effects.

Oestrogen-progesterone modulation

Regular acupuncture across the menstrual cycle normalises the oestrogen-progesterone dynamic in the luteal phase, reducing the severity of hormonal fluctuation that triggers PMS symptoms.

Anxiety and mood stabilisation

For the anxiety and emotional dysregulation component of PMS, acupuncture's HPA axis regulation and GABA-modulating effects provide significant relief.

Evidence Base

What the Research Says

Acupuncture for PMS has consistent clinical evidence. A 2018 systematic review found acupuncture significantly reducing total PMS symptom scores, with particular efficacy for mood symptoms and physical pain. Studies specifically on PMDD (severe PMS) show acupuncture producing comparable symptom reduction to SSRIs, without the sexual side effects, weight changes, and discontinuation syndrome associated with pharmacological treatment. The WHO lists PMS as an indication for acupuncture.

Patient Experience
I was genuinely a different person for 10 days every month. My partner could tell the day my PMS started. After 3 cycles of acupuncture with our team, my luteal phase symptoms reduced by about 70%. My mood is now consistent across the whole month for the first time I can remember.

Your Questions Answered

Yes. PMDD responds to the same Liver Qi stagnation protocol, with additional Heart Shen treatment for severe emotional dysregulation. PMDD is more complex and takes longer — typically 4–5 cycles for meaningful improvement.
