Uterine fibroids (leiomyomas) are benign smooth muscle tumours of the uterus affecting an estimated 70–80% of women by age 50, though most are asymptomatic. Symptomatic fibroids produce heavy menstrual bleeding, prolonged periods, pelvic pain and pressure, urinary frequency, and subfertility — significantly impacting quality of life.
Standard management is either watchful waiting (for small, asymptomatic fibroids) or surgical intervention — myomectomy or hysterectomy. Medical management with GnRH agonists reduces fibroid size temporarily but produces menopausal side effects and is not a long-term solution. Many women find themselves in an inadequately managed middle ground: too symptomatic to wait, not yet ready for surgery.
Fibroids are oestrogen-dependent — they grow in the presence of oestrogen and regress after menopause. This hormonal dependency is the mechanism through which acupuncture's oestrogen-modulating effect can influence fibroid behaviour and, more reliably, fibroid-related symptoms.
In TCM, fibroids are Blood stasis masses — zheng jia — formed by the accumulation of stagnant Blood in the uterus over time, combined with Phlegm-Damp in many presentations.

Pelvic pain and pressure
