Understanding the Condition

When the Bladder Won't Wait

Overactive bladder (OAB) is characterised by urinary urgency — a sudden, compelling desire to urinate that is difficult to defer — typically accompanied by urinary frequency (more than 8 times per day), nocturia (waking at night to urinate), and urgency incontinence in many patients. It affects an estimated 11–16% of adults and significantly impairs quality of life, sleep, work productivity, and social confidence.

The underlying mechanism is detrusor overactivity — inappropriate, involuntary contractions of the bladder muscle (detrusor) during the filling phase, when it should remain relaxed. The neurological trigger is heightened C-fibre afferent activity from the bladder wall sending excessive urgency signals to the pontine micturition centre in the brainstem.

Standard treatment — antimuscarinics (oxybutynin, tolterodine) and beta-3 agonists (mirabegron) — reduces detrusor contractility pharmacologically but produces anticholinergic side effects (dry mouth, constipation, cognitive impairment in elderly patients) that limit tolerability. Many patients discontinue medication within 3 months. For medication-refractory OAB, botulinum toxin injection and sacral neuromodulation are next-line options — the latter sharing its mechanism with acupuncture.

Sacral neuromodulation

Acupuncture at BL-32 and BL-33 — overlying the second and third sacral foramina — directly stimulates the S2-S3 sacral nerve roots that govern detrusor activity. Detrusor C-fibre suppression
