Tinnitus is the perception of sound in the absence of an external acoustic stimulus — ringing, buzzing, hissing, whistling, or roaring, in one or both ears. Severe tinnitus is associated with anxiety, depression, and social withdrawal.

The mechanisms are complex: cochlear damage (noise exposure, ototoxic drugs, age-related hearing loss), central auditory pathway dysregulation, vascular conditions, temporomandibular disorder, cervical dysfunction, and in many cases, no identifiable structural cause. Standard ENT management — sound therapy, cognitive behavioural therapy, tinnitus retraining — helps patients habituate but does not reduce the underlying signal.

TCM Perspective

In TCM, tinnitus is primarily a Kidney condition — the Kidney "opens into the ear" and Kidney deficiency (the most common pattern, particularly in older patients) produces the thin, high-pitched tinnitus of deficiency.
