Urticaria (hives) is characterised by raised, itchy wheals that appear suddenly on the skin, lasting from minutes to hours, driven by mast cell degranulation and histamine release. Acute urticaria is typically triggered by allergens, infection, or medication. Chronic spontaneous urticaria (CSU) — hives occurring most days for 6+ weeks without an identifiable trigger — affects 0.5–1% of the population and significantly impacts quality of life.

In CSU, the chronic mast cell hyperreactivity has an autoimmune basis in approximately 40–50% of patients — IgG antibodies activating mast cells directly. Standard treatment is antihistamine escalation, omalizumab (anti-IgE biologics), and in severe cases, immunosuppressants. Antihistamines manage symptoms but do not reduce the underlying mast cell hyperreactivity. CSU typically runs a course of months to years before spontaneous remission.

TCM Perspective

In TCM, urticaria is Wind in the skin — the sudden appearance and disappearance of wheals is the classic Wind pattern (which "moves and changes").
