An Autoimmune Attack on the Skin's Colour Cells

Vitiligo is an autoimmune condition in which the immune system destroys melanocytes — the cells responsible for skin pigmentation — producing well-defined white patches on the skin. It affects approximately 1–2% of the population globally, with higher prevalence in India, and carries significant psychological and social impact, particularly in Indian cultural contexts where skin tone is culturally weighted.

The condition is driven by aberrant cytotoxic T-cell activity targeting melanocytes, combined with oxidative stress that sensitises melanocytes to immune destruction. Standard treatment — topical corticosteroids, tacrolimus, phototherapy (PUVA or NB-UVB) — can stimulate repigmentation in active vitiligo but is slow, partially effective, and associated with side effects with long-term use. Segmental vitiligo (unilateral, following a dermatome) behaves differently from generalised vitiligo and has better surgical outcomes.

TCM Perspective

In TCM, vitiligo — bai dian feng ("white spot wind") — reflects Wind invasion with Qi and Blood deficiency, producing the loss of normal pigmentation as Blood fails to nourish the skin. Kidney deficiency is the constitutional root in chronic presentations.
