Benign paroxysmal positional vertigo is the most prevalent vestibular disorder globally, accounting for approximately 20% of all vertigo presentations. It is caused by the dislodgement of otoliths — calcium carbonate crystals that normally reside in the utricle of the inner ear — into one of the semicircular canals. When displaced otoliths move within the canal fluid in response to head position changes, they generate an inappropriate endolymph current, producing the characteristic brief (20–60 second) but intense burst of rotational vertigo triggered by specific head movements: rolling over in bed, looking up, bending forward, or rising from lying.

BPPV is diagnosed clinically by the Dix-Hallpike test, which reproduces the characteristic nystagmus (involuntary eye movement) associated with each canal variant. The posterior canal is most commonly affected (80–90%), with horizontal and anterior canal variants less common.

The Epley manoeuvre — a sequence of head positions designed to guide displaced otoliths back to the utricle — is the gold-standard first-line treatment, resolving approximately 80% of posterior canal BPPV in 1–2 sessions. The critical clinical problem is recurrence: BPPV recurs in approximately 50% of patients within 5 years, and up to 30% within the first year. Recurrence is driven by the same predisposing factors that caused the initial episode — vitamin D deficiency, vestibular nerve micro-trauma, and most importantly in clinical practice, the constitutional vulnerability that TCM identifies as Phlegm-Damp accumulation and Kidney deficiency.
