The pain of renal colic — produced as a stone migrates from the kidney into the ureter — is among the most severe in medicine. It is characterised by colicky flank pain radiating to the groin, nausea, vomiting, and haematuria. Standard management for acute colic is analgesia (NSAIDs, opioids) and alpha blockers to facilitate stone passage. Stones larger than 5mm may require ureteroscopy or lithotripsy.

Beyond acute management, the high recurrence rate makes prevention the most clinically important long-term goal. Dietary modification (reduced sodium, oxalate, and animal protein; increased hydration and citrate) addresses the metabolic environment, but compliance is challenging and stones still recur in many patients.
