When You Become Aware of Your Own Heart

Palpitations — the subjective awareness of the heartbeat, perceived as racing, fluttering, pounding, or irregular — are one of the most common cardiac complaints and one of the most anxiety-provoking. They may arise from genuine cardiac arrhythmia (ectopic beats, SVT, AF), from non-cardiac causes (anaemia, thyroid dysfunction, hypoglycaemia, caffeine, anxiety), or most commonly, from a combination of heightened cardiac sensitivity and autonomic dysregulation that makes normal heartbeats feel abnormal.

In Mumbai's population, stress-related palpitations are ubiquitous. The HPA axis activation of chronic stress increases sympathetic cardiac tone, elevates resting heart rate, sensitises the cardiac interoceptive network, and creates the conditions in which palpitations are both more frequent and more distressing. Anxiety about palpitations compounds the problem — the worry activates the sympathetic system further, creating a vicious cycle.
